4/11/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of JaâCarei. Upon arrival to the home setting, JaâCarei appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C engaged JaâCarei in a discussion to determine behaviors since the last session. Ms. Cosby discussed with QMHP-C on how he continues to be disrespectful towards her and his siblings. 2. Behavior: JaâCarei participated in the session and needing several prompting from QMHP-C due to him becoming aggressive when discussing information presented by his mother. JaâCarei indicated to QMHP-C he utilizes the skills that are being taught to him by his mother; however, at times, his mother just takes him to another level and he just ignores her and that is something that she does not like. 1. Intervention: QMHP-C initiated individual session to review social skills that will assist JaâCarei to interact with his family appropriately while avoiding anger outbursts. QMHP-C acknowledged JaâCarei as he was able to share his thoughts that pertain to becoming verbally aggressive. QMHP-C demonstrated verbal ques that can be identified as threatening and provoking. QMHP-C asked JaâCarei to share his thoughts about the exercise. QMHP-C asked JaâCarei to mirror QMHP-C's demonstration in order to gain insight of how to avoid provoking or threatening behaviors. QMHP-C listened as JaâCarei shared his thoughts. 2. Intervention: QMHP-C presented JaâCarei engaged client in a therapeutic activity via QMHP-Csâ cellular device. QMHP-C explained to client the purpose of this was to assist him with learning about himself as an individual (checklist with behaviors and he was instructed to check those behaviors that he lacks and display). QMHP-C instructed JaâCarei to complete the activity and to share his thoughts about the exercise presented via QMHP-Csâ cellular device. 1. Response: JaâCarei was receptive of the session as evident of his participation with identifying the verbal ques in communication of peers and family members whom display provoking behaviors towards him. He appeared to have an understanding of how to utilize appropriate verbalizations to avoid anger outbursts in the home and community. He stated that he has difficulty ignoring his peers or provoking behaviors. 2. Response: JaâCarei participated in the activity provide by QMHP-C during the session and provided input on its effectiveness to helping gain insight of how he can control his anger towards his mother and others in the community. He stated to QMHP-C that it doesnât matter how much good he does, his mother is always going to report to QMHP-C that he is not demonstrating positive behaviors
